Informed Consent

Informed Consent

All patients need to be provided with an informed consent form. This form needs to provide information about participation in the database and must sign an ICMS approved informed consent document indicating their consent for their data to be entered into the database. Documentation of assent and of parent or legal guardian permission of minor participants, and consent for adult participants, must be maintained at the center where the participant, or their parent or legal guardian provided consent to participate. To confirm that participants have given consent to participate in the database, the first form submitted for a participant includes confirmation that the participant signed the informed consent document.
